Profhilo Structura works in the adipose layer, not the dermis. This single fact underpins everything that follows. When we lose facial fat with age, that loss happens in specific compartments: the submalar (cheek), the buccal (below the cheekbone), and areas around the jawline. These aren't areas where traditional fillers live. And hydrating the skin surface doesn't restore what's gone.

Structura is engineered to bio-remodel these superficial fat pads by stimulating the adipocytes (fat cells) themselves to regenerate. This is regenerative medicine, not volume replacement. The face is strengthened from within because the tissue itself is behaving more youthfully, not because we've injected a gel underneath it.

The formulation is key here. Profhilo Structura uses patented NAHYCO technology to stabilise both high and low molecular weight hyaluronic acid without chemical cross-linkers. This matters because the product needs to remain bioavailable within the fat compartment long enough to trigger that regenerative response. At 90mg per 2ml syringe, it's significantly more concentrated than original Profhilo, and that concentration is deliberate.

Structura versus original Profhilo

Original Profhilo is a skin booster. It's injected with a needle into the dermis in a specific pattern, and it spreads through the tissue to deliver deep hydration and stimulate collagen and elastin. It's brilliant for skin quality, luminosity, and fine lines. But it doesn't target loss of structural support or volume in the fat compartments.

Profhilo Structura, by contrast, is delivered by cannula into the subcutaneous layer. The cannula allows us to place the product precisely within the superficial fat pads that have atrophied with age. From there, it works on the adipocytes themselves, not on skin hydration.

In my clinic, patients often ask if they can combine the two. The answer is yes, but with intention. Original Profhilo addresses skin quality and fine lines. Structura addresses volume loss and laxity in the mid-face. A patient with hollowed cheeks and poor skin texture might benefit from both, spaced appropriately. How Structura differs from dermal fillers

This is where many clinics and patients get confused. Dermal fillers physically replace lost fat with synthetic hyaluronic acid gel. You inject the filler, and volume is added immediately. The patient sees a result on the day. Structura does something altogether different.

Structura doesn't add volume in that immediate sense. It teaches the tissue to behave more youthfully. The results unfold over weeks as the adipocytes respond. Clinical trials showed that 95% of patients noticed mid-face improvement after completion of a two-part treatment course. But that improvement is subtle and biological, not the obvious plumping you'd see with a traditional filler.

This matters for expectations. If a patient wants their cheeks lifted by 2mm by Friday, Structura isn't the answer. It's less suited to patients with primarily superficial skin concerns (fine lines, pigmentation, dullness) where original Profhilo would be more appropriate. And it's not a first-line treatment for patients seeking dramatic volume restoration; in those cases, traditional fillers or biostimulators like HarmonyCa may deliver faster, more visible results.

Treatment protocol and realistic timelines

Profhilo Structura is delivered via two treatments spaced 4-6 weeks apart. This interval allows the adipocytes to respond to the first treatment before the second application. It's not optional; the two-part protocol is fundamental to how the product works.

Currently, Structura is injected preauricularly (in front of the ear) into the submalar fat pads. The cannula is placed in the subcutaneous layer, targeting the dynamic superficial fat pads that sit between the skin, connective tissue, muscle, and deeper fat layers. The procedure takes around 20-30 minutes.

Expect modest swelling and tenderness for 24-48 hours. Some clients describe a mild ache, similar to a muscle soreness. There may be slight bruising, though cannula placement minimises this compared to needle-based injections. Most patients return to normal activity immediately.

Results aren't visible on day one. The regenerative response takes time. Most clients notice subtle improvement in mid-face definition and support over 3-4 weeks. This continues to improve after the second treatment. The effects last around 6-9 months, after which maintenance treatments are needed if the result is to be sustained.

Honest expectations and limitations

I want to be clear about what Structura isn't. It's not a non-surgical facelift. It won't transform a severely aged or ptotic face into a youthful one. It won't replace the need for other treatments if skin quality is poor or fine lines are prominent. And it's not a quick fix for dramatic volume loss.

What it is: a sophisticated option for bio-remodelling specific fat compartments that have atrophied. It's part of the shift in aesthetics away from volume replacement and toward regenerative approaches. In my clinic, I often combine it with original Profhilo for skin quality, or with subtle filler in other areas where volume restoration is appropriate.

The result, when it works well, is a face that looks naturally refreshed, structurally supported, and less hollowed. Not dramatically different, but noticeably better. And that's the point.

Safety and practitioner standards

Structura is a relatively new product, which means it's crucial that it's administered by a practitioner with proper training and anatomical knowledge. In the UK, practitioners performing injectable treatments like this now require at minimum a Level 7 qualification (equivalent to Master's degree standard) in clinical aesthetic injectable therapies. This isn't a regulation many clinics follow, but it should be.
