Profhilo for the neck and décolletage: realistic expectations

Why the neck shows age faster than the face

The neck and décolletage are chronically neglected in skincare routines. Most people apply sunscreen to the face, but stop short of the jawline. The skin on the neck is thinner, with fewer sebaceous glands and less natural moisture production. Over years, this neglect combines with constant movement, sun exposure, and loss of dermal collagen and elastin to create visible ageing that often lags behind facial concerns by a decade.

In my clinic, I see clients who have invested heavily in facial treatments but are shocked to find their neck looks 10 years older than their face. By the time they book a consultation, the neck often shows fine horizontal lines, crepiness (a creped or papery texture), and mild loss of firmness. Some also develop vertical platysmal bands (the muscle cords running down the front of the neck), though those require a different treatment entirely.

Profhilo is an excellent option for addressing the first three concerns: hydration, crepiness, and mild laxity. It is not, however, a treatment for deep wrinkles, established platysmal bands, or surgical-grade skin sagging.

How Profhilo works: the mechanism

Profhilo is a hybrid hyaluronic acid (HA) treatment. Each 2 ml syringe contains 32 mg of high-molecular-weight HA and 32 mg of low-molecular-weight HA suspended in saline. This combination is designed to do two things simultaneously: hydrate the dermis immediately (especially the high-molecular-weight component), and stimulate the skin's own production of collagen and elastin over the following weeks (the low-molecular-weight component).

It is a bio-remodelling treatment, not a dermal filler. It does not add volume, does not change your face shape, and does not fill lines the way a filler does. Instead, it works underneath the skin surface to improve skin quality: texture, hydration, elasticity, and firmness. Results build progressively over 8 to 12 weeks as the skin's own collagen and elastin production responds to the HA stimulus.

Injection technique on the neck: the BAP Neck protocol

Profhilo on the neck uses a specific injection technique called BAP Neck (Bio Aesthetic Point Neck). Rather than deep volume placement, the product is injected into the middle-to-deep dermis using a 29-gauge needle. The practitioner creates a series of microwheals (small depots of 0.2 ml each) arranged in three vertical lines across the front of the neck and décolletage.

This differs from facial Profhilo, where injections are placed around the cheekbones, temples, and jawline. On the neck, the anatomy is flatter, the skin is thinner, and the goal is broad hydration and texture improvement rather than targeted lifting or volumisation. The vertical-line approach ensures even distribution of the product across the front and sides of the neck.

The injection points are spaced carefully to avoid the thyroid gland and major vessels. A skilled practitioner will assess the skin on the day and may adjust the pattern slightly depending on where crepiness is most pronounced or where the client reports the most laxity.

The two-session protocol: why four weeks apart is non-negotiable

Profhilo results are based on a two-session course, four weeks apart. This is not a sales tactic. The protocol is built into all published clinical evidence, and it reflects a genuine biological principle: the first session primes the skin; the second session deepens and locks in the bio-remodelling effect.

After the first session, you will notice improved hydration and a brighter appearance within two to three weeks. But the collagen and elastin stimulation takes time. By week four (your second session), the skin is primed and ready to respond more robustly to a second dose of HA. The second session is then assessed at weeks 4, 8, 12, and 16, with peak results (increased elasticity and firmness) arriving around weeks 6 to 8 after the second injection.

If you skip the second session, you will experience improvements in hydration and fine texture, but you will miss the deeper, more durable collagen-building effect. In my clinic, clients who have completed both sessions consistently report better results than those who stopped after one.

What Profhilo improves on the neck

Here is what Profhilo genuinely does for the neck and décolletage:

• · Improves hydration: the skin feels plumper, smoother, and looks fresher within two to three weeks.
• · Reduces fine crepiness: the papery, creped texture softens and becomes less pronounced over 8 to 12 weeks.
• · Mildly improves skin laxity: clients report that the skin feels firmer and slightly more taut, though this is a subtle improvement, not a dramatic lift.
• · Refines fine horizontal lines: shallow lines improve with better hydration and elasticity, but deep, entrenched horizontal wrinkles do not disappear.
• · Enhances overall skin quality: the neck looks brighter, smoother, and more refined without looking overdone.

Results are most visible in clients with early signs of ageing (fine lines, crepiness, loss of glow) rather than in those with advanced sagging or deep wrinkles. If your main concern is significant loose skin or deep horizontal creases, Profhilo alone may not meet your expectations, though it can be part of a combination approach.

What Profhilo does not do

It is crucial to be clear about Profhilo's limitations, because disappointed clients often result from unrealistic expectations, not from treatment failure.

• · Deep horizontal lines: if you have deeply etched horizontal wrinkles running across the neck (the kind you can see and feel clearly), Profhilo will not erase them. It softens and improves fine lines, not deep creases.
• · Platysmal bands: the vertical cords running down the front of the neck are caused by muscle contraction, not skin quality. Profhilo does not address muscle. Anti-wrinkle injections (botulinum toxin) or surgical approaches are the correct treatments for established bands.
• · Significant skin sagging: if your primary concern is loose, hanging skin or jowling that extends to the neck, Profhilo will not provide the lift you need. You would be better served by a combination of treatments or, in severe cases, surgical options.
• · Volume loss: Profhilo is not a volumiser. If the neck has lost significant fat and appears hollow, dermal filler or biostimulators like HarmonyCa may be more appropriate.

I often see clients who hope Profhilo will transform their neck overnight. The honest truth is that Profhilo is a skin-quality treatment. It is excellent at what it does, but it is not a deep-wrinkle or sagging-skin solution. Knowing this before you book prevents disappointment.

Timeline, results, and maintenance

Here is a realistic results timeline after your two-session course:

• · Weeks 1 to 3 after session one: improved hydration and a brighter, fresher appearance.
• · Week four: second session (the skin is now primed for a deeper response).
• · Weeks 5 to 8 after session two: collagen and elastin stimulation peaks; you will notice improved firmness, elasticity, and texture.
• · Weeks 8 to 12: results continue to refine and settle; skin looks smoother, crepiness is reduced, fine lines are softened.
• · Week 16 and beyond: results plateau and remain stable.

Profhilo's results typically last 6 to 9 months. After that, the hyaluronic acid is gradually metabolised by the body, and the collagen-building stimulus fades. To maintain results, most clients return for a single maintenance session (or sometimes two sessions, depending on individual response) annually or every six months. This is far less frequent than dermal fillers, which typically require top-ups every 9 to 12 months.

Is Profhilo right for your neck?

Profhilo is an excellent choice if you have early-to-moderate signs of neck ageing: fine horizontal lines, crepiness, loss of hydration, and mild laxity. It is also worth considering if you want to prevent further ageing on a neglected neck; treating early is always more effective than treating late.

Profhilo is not the right choice if your primary concern is deep wrinkles, established platysmal bands, or significant skin sagging. You may benefit from a combination approach (for example, Profhilo for overall skin quality plus anti-wrinkle injections for the platysmal bands), but a consultation will help clarify what is achievable and what requires a different treatment.
