Polynucleotides for under-eyes: the filler alternative

The under-eye area is unforgiving. Thin skin, delicate anatomy, and high visibility make it the hardest region to treat well. Dermal filler can go wrong in ways that are visible every time you look in a mirror. Why the under-eye area is so unforgiving

The under-eye region, particularly the tear trough (the groove running from the inner corner toward the cheek), is a study in fragility. The skin here is thinner than almost anywhere else on the face. There is limited lymphatic drainage, meaning swelling that occurs can linger for days. The area sits directly over bone with minimal soft tissue buffer. Blood vessels are superficial and numerous. And because we look at each other's eyes constantly, any irregularity is instantly visible.

These factors combine to make the tear trough one of the highest-risk zones for dermal filler complications. I have seen clients present with problems caused by filler placed elsewhere that migrated down into the under-eye area, as well as filler placed there directly that went wrong.

The problem with filler under the eyes

Hyaluronic acid filler, the most common choice, is placed below the skin surface. In the under-eye area, the margin between "deep enough to look natural" and "too superficial" is perhaps 1 millimetre. Place it too high and you risk the Tyndall effect: a bluish or grayish discoloration where light scatters through the product at a shallow depth. The effect can take weeks to become apparent and is difficult to correct once it appears.

The most common complication I see reported is prolonged swelling. The tear trough's poor lymphatic drainage means that even a perfectly placed filler can trigger days of puffiness. Clients return to work looking "done" or "overdone" when in fact they simply have temporary inflammation. This resolves, but the anxiety it causes is real.

There is also the fundamental issue that filler adds volume. The problem most clients present with is not that they lack volume in the tear trough; it is that the skin there is thin, dehydrated, and crepey, often with fine lines and a shadowy appearance. Adding a gel does not fix those things. It may obscure them temporarily, but it does not rebuild the tissue.

How polynucleotides work differently

Polynucleotides are long-chain DNA fragments that work through a regenerative mechanism, not a volumising one. When injected intradermally, they bind to cell-surface receptors and trigger a biological repair cascade. Fibroblasts, the cells responsible for making collagen, become more active. The extracellular matrix (the scaffold that gives skin its structure) rebuilds. Blood supply to the treated tissue improves. Over time, skin hydration increases, elasticity recovers, and the overall tone improves.

This is not a temporary effect. You are not injecting a substance that sits in the skin and gradually metabolises. You are triggering your own cells to do repair work. A typical polynucleotide protocol for the under-eyes involves a loading phase: 3 to 4 sessions spaced 2 to 4 weeks apart. The first session primes the repair response. The second and third sessions build on that foundation. The fourth session, if recommended, consolidates the result.

During the loading phase, you should avoid aggressive skincare, heavy sun exposure, and alcohol for 48 hours after each treatment. The under-eye area may feel slightly tender or show mild redness for a few hours. This settles quickly. Unlike filler, there is no significant swelling or bruising in most cases, though individual responses vary.

After the loading phase is complete, results persist for approximately 6 to 9 months depending on individual factors such as age, skin type, lifestyle, and skincare consistency. To maintain the improvement, most clients return for a single maintenance session every 6 to 9 months. This is far less frequent than filler, which typically requires topping up every 3 to 4 months.

What results actually look like

The goal with polynucleotides for the under-eye area is not dramatic. You are not aiming for a before-and-after photograph with an obvious difference. You are aiming for clearer, brighter, more hydrated skin. Fine lines soften. The crepey texture smooths. The skin appears more resilient and less shadowy. Your eyes look more rested.

Polynucleotides work best on dark circles caused by thin, dehydrated skin (where the darkness comes from poor skin texture and light absorption). Because the mechanism is regenerative rather than volumising, there is no product sitting in place that could migrate, cause foreign-body reactions, or create long-term visibility problems.

Not all practitioners offering polynucleotide treatments have the same level of training or understanding of the mechanism.
