Lip flip vs lip filler: when each one is actually right

What is a lip flip, and how does it work?

A lip flip is a small dose of anti-wrinkle injection (typically 2-4 units) placed along the border of the upper lip. The injection relaxes the orbicularis oris muscle, which normally holds your upper lip in a slightly inward position. When this muscle releases, your upper lip rolls gently outward, creating the appearance of a fuller, more defined lip without any actual volume being added.

The effect is subtle and works best for specific concerns. Because you're not adding filler, there's no swelling, no bruising, and no risk of overfilling. The result looks natural because it's simply revealing more of your natural lip surface.

Results begin to show within 3-5 days, with full results visible around the two-week mark. The effect lasts 6-8 weeks, after which the muscle gradually regains function and the lip returns to its natural position. This is important: a lip flip is not permanent, and it's not reversible in the way filler is. Your body simply metabolises the anti-wrinkle product naturally over time.

What is lip filler, and how long does it last?

Lip filler adds actual volume to the lips. Most lip fillers used in aesthetics practice contain hyaluronic acid, a sugar derivative naturally produced by your body that can attract and retain up to 1,000 times its own weight in water. This makes it an excellent choice for lip augmentation because it's biocompatible and integrates seamlessly with your own tissue.

When injected into the lips, hyaluronic acid fillers create noticeable volume increase in either the upper lip, lower lip, or both. Most are infused with lidocaine to reduce discomfort during the procedure. Results are immediate, although some initial swelling is normal and settles over 24-48 hours. Once settled, you'll see your true result.

The duration of hyaluronic acid fillers is 6-12 months, depending on the specific product, injection technique, and how your individual body metabolises the filler. A real advantage of HA filler is reversibility: if you're unhappy with the result, or want to adjust the volume, hyaluronidase can be injected to dissolve the filler. This makes lip filler a relatively low-risk way to explore what fuller lips feel like on you.

When a lip flip is genuinely the right answer

I recommend a lip flip in three scenarios:

• · You have a gummy smile, where your upper lip disappears or curls inward when you smile. The lip flip keeps the upper lip flipped outward, so the gum exposure is minimised and the lip looks fuller during the smile.
• · Your vermillion border (the natural edge of your lips) is thin, and you want definition without volume. The flip makes the border more prominent by rolling the lip outward, accentuating your natural lip shape.
• · You want a lower-commitment first step. A six to eight-week trial is a sensible way to see how fuller lips feel and look on you before committing to six to twelve months of filler.

I also use a lip flip in my own clinic as a complement to filler. The filler provides volume and definition, while a small dose of anti-wrinkle in the orbicularis oris enhances projection and reduces any residual upper lip curl. This combination often gives the most balanced, natural-looking result.

When lip filler is the right choice

Choose filler if you're seeking noticeable volume increase in the upper and/or lower lip. This might be because your lips are naturally thin, because you want more definition in the lip border, or simply because you prefer the look of fuller lips. Filler is also the choice if you want results that last longer than eight weeks, since anti-wrinkle injections need to be redone every 6-8 weeks to maintain effect.

Filler is also appropriate if you want to build your lip shape gradually. A skilled practitioner will typically start with a modest volume at your first appointment, allow two weeks for the swelling to fully settle, then assess whether you want to add more at a follow-up visit. This conservative approach helps you land on a look that feels right for you, rather than over-filling at the first go.

Combining both for the best result

In my clinic, I find that many patients achieve their ideal lip aesthetic by combining filler with a lip flip. The filler creates the volume and structure, while the anti-wrinkle injection enhances the projection of the upper lip and softens any remaining inward curl. The two treatments work together: the filler does the heavy lifting, and the flip adds definition and subtle enhancement.

If you're considering both, the timing matters. I typically inject the filler first, allow two weeks for full settling, then add the lip flip if you want an extra boost of projection. Alternatively, you can have both at once, though this increases initial swelling and makes it harder to assess your true filler result until the swelling subsides.

A short safety note

Both anti-wrinkle injections and lip fillers are prescription-only medicines in the UK. This means they must be prescribed by a registered prescriber (a doctor, dentist, or nurse prescriber) only after a face-to-face consultation. Since June 2025, the NMC has enforced a binding standard requiring nurse and midwife prescribers to see you in person before issuing any cosmetic injectables prescription.

Recently, there have been confirmed cases of counterfeit or unlicensed botulinum toxin in the UK market. Always check that your practitioner is regulated by the NMC, GMC, or GDC and uses only licensed products from reputable pharmaceutical suppliers. If the price seems too good to be true, it probably is.
