Why diagnosis matters more than the product

In my clinic, the first conversation about a double chin is never about Aqualyx or HIFU. It's about what's creating the appearance. A double chin results from three anatomical layers: skin, fat, and muscle tone. With age, the skin loses collagen and elasticity, the fat pad enlarges due to genetic predisposition and gravity, and the anterior belly of the digastric muscle weakens. None of these always correlate with weight. I've seen people at a healthy weight with a pronounced double chin because of genetics or posture, and people carrying extra weight with minimal submental fullness because of good skin elasticity and muscle tone.

This matters because a treatment that dissolves fat will disappoint someone whose main problem is loose skin. Similarly, a tightening treatment will do little for someone carrying significant fat and expecting volume loss. The right treatment depends on what you're actually looking at.

How Aqualyx fat dissolving works

Aqualyx is an injectable containing phosphatidylcholine and deoxycholate. When injected into subcutaneous fat, these molecules interact with fat cell membranes, producing a controlled inflammatory response. The fat cell membrane breaks down, the cell contents leak out, and the body's immune system clears the debris through phagocytosis. Over the following weeks, the area remodels and the fat volume reduces.

Aqualyx is not a weight-loss treatment. It's a body-contouring tool for people at or near their goal weight who have localised, stubborn fat deposits that haven't responded to diet or exercise. It works best when you already have good skin elasticity, because the skin needs to contract down onto the smaller fat pad beneath. If your skin is already loose or sagging, Aqualyx will reduce the volume underneath but won't tighten the skin above it, leaving you with a deflated appearance.

Candidates for Aqualyx typically have stable weight, clear localised fat (you can pinch it and feel it as distinct from surrounding tissue), adequate skin elasticity, and realistic expectations. If you're still losing weight, or your skin is visibly lax, Aqualyx alone may not deliver the result you're hoping for.

Important contraindications for Aqualyx

Aqualyx is not suitable if you are pregnant or breastfeeding, have an active autoimmune condition, are diabetic, have severe liver or kidney disease, or have a history of anaphylactic reactions. Active skin infection at the injection site is also a contraindication. These are not negotiable; they exist because the treatment relies on a controlled inflammatory response, and these conditions either interfere with that process or put you at risk.

How HIFU works and why it addresses skin, not just fat

HIFU stands for high-intensity focused ultrasound. The device delivers focused ultrasonic energy to precise depths beneath the skin, creating thermal zones within the fat compartment. At temperatures around 58°C, the ultrasound causes fat cell membranes to rupture, destroying the cells, while simultaneously stimulating the deeper skin layers to produce new collagen and elastic fibres. This dual action is why HIFU appeals to people with both fat and loose skin: you're not just dissolving fat, you're also tightening.

Results develop progressively over 4 to 12 weeks as collagen remodels. HIFU works particularly well for mild to moderate double chins where the concern is loose or slightly sagging skin combined with moderate fat. The focused ultrasound spares the skin surface and underlying muscle, which is why downtime is minimal.

The catch: HIFU has limits with very large fat deposits. If your double chin is mostly volume from fat, rather than laxity, HIFU alone may not deliver as much reduction as you'd expect. The skin tightening is real, but the fat reduction is modest compared to Aqualyx. Most people see significant results with one or two sessions spaced 3 to 6 months apart, though the number needed varies by individual.

Aqualyx versus HIFU: who suits which

The choice between these two comes down to what you're treating.

• · Choose Aqualyx if your double chin is primarily fat with good skin elasticity. You can pinch the fullness and feel it as fat; the skin above snaps back when you release it. You're at a stable weight and realistic about the fact that Aqualyx won't tighten loose skin.
• · Choose HIFU if your double chin combines moderate fat with visible skin laxity or if tightening the lower face and jawline is as important as reducing fullness. You want a single session with no downtime and don't mind waiting 4 to 12 weeks for results.
• · Choose neither if your double chin is mainly loose, sagging skin with minimal fat. This may benefit from a different approach, such as skin-tightening treatments or addressing jaw projection, rather than volume reduction.
• · Choose neither if you're carrying significant fat and expect a dramatic change from one session. Both treatments are designed for mild to moderate fullness, not major contouring.

The honest limits of both treatments

In my experience, the biggest disappointments arise from mismatched expectations. Someone with very loose skin under the chin books Aqualyx expecting to lose the fullness and tighten the skin. Aqualyx dissolves the fat, the skin contracts a little, but the laxity remains. Or someone with significant fat deposits books HIFU hoping for major volume loss. HIFU tightens the skin nicely, but the fat pad is still there.

Neither treatment is a substitute for addressing poor jaw projection, chin position, or neck laxity at scale. If your double chin is partly due to a recessed chin or weak mandible, volume reduction alone won't fix the underlying structure. That conversation requires a different set of treatments, possibly including chin augmentation with filler, and may benefit from consultation with someone trained in facial anatomy and proportion.

A short safety note: regulatory clarity has changed recently in the UK. Since 1 June 2025, remote prescribing of cosmetic injectables (including Aqualyx) is prohibited. You must attend a face-to-face consultation with a qualified prescriber before any treatment. This is a good safeguard; it ensures proper assessment and contraindications screening. Reputable clinics have always done this, but it's now a legal requirement.

What happens during a proper consultation

A thorough assessment involves more than a glance. A practitioner should examine the under-chin area, assess the amount of fat and skin laxity, check skin elasticity by gently pinching, evaluate your jawline and chin projection, and ask about your medical history, current weight stability, and what you're hoping to change. They should discuss which treatment, if any, fits your anatomy and goals, and be honest if neither is ideal for you.

This is also the moment to ask about aftercare, realistic timescales, and what you can expect to see. If a practitioner promises dramatic results from a single session, or doesn't ask questions about your skin elasticity and fat distribution, that's a red flag. Good practitioners take time; they don't rush diagnosis into treatment.

The right double chin treatment isn't about choosing between two popular options. It's about understanding what's actually causing your double chin, and selecting something that matches that diagnosis. That requires clinical assessment, honesty about limits, and a practitioner who's willing to say "nothing we do will fix that" if that's the truth.
