Vitamin B12 Injections: When They Actually Work
B12 injections have become a wellness staple, marketed as an instant energy fix for anyone feeling tired. The reality is more nuanced. For some people, injectable B12 is genuinely medically necessary. For others, it's an expensive placebo when a tablet would do the same job, or when they don't need supplementation at all. Here's how to know which category you fall into.
The short version
B12 injections are genuinely essential for people with pernicious anaemia, certain neurological conditions, or malabsorption issues from surgery or gut disease. For everyone else, including most people seeking "energy injections" at wellness clinics, high-dose oral B12 works just as well for normalising blood levels. If your B12 isn't actually low, neither route will give you more energy. The honest answer: get tested first, understand your result, then decide on the delivery method that makes clinical sense.
Who genuinely needs injectable B12
The most common cause of vitamin B12 deficiency in the UK is pernicious anaemia, an autoimmune condition where the body attacks the cells that produce intrinsic factor (a protein needed to absorb B12 from food). This isn't related to diet. If you have pernicious anaemia, you'll need hydroxocobalamin injections every two to three months for the rest of your life, because your gut simply cannot absorb the vitamin efficiently through normal routes.
Neurological symptoms take this a step further. If B12 deficiency has affected your nervous system (numbness, tingling in hands or feet, balance problems, cognitive changes), the NHS recommends more intensive treatment: injections on alternate days until symptoms stop improving, then maintenance injections every two months. This isn't something to experiment with. Neurological damage from B12 deficiency can become permanent if not treated promptly and aggressively.
Patients with severe symptoms, critically low blood levels, or significant neurological deficits should receive intramuscular administration to ensure rapid replenishment and prevent irreversible consequences. This is established clinical guidance, not a matter of preference.
The other clear-cut group is people who've had certain types of bariatric surgery. Gastric bypass and similar procedures can permanently alter B12 absorption. While some forms of bariatric surgery allow for oral supplementation if the patient prefers, intramuscular B12 is generally the preferred treatment according to current NHS regional guidelines.
The science of oral versus injectable
This is where the marketing around "energy injections" starts to fall apart. The clinical evidence is now fairly robust: for normalising serum B12 levels, oral and intramuscular supplementation are equivalent after one to four months. Multiple systematic reviews have confirmed this. The idea that injections are inherently "stronger" or "more effective" than tablets isn't supported by the research.
The mechanism is interesting. We've always known that B12 absorption in the gut depends on intrinsic factor, which is why pernicious anaemia (where intrinsic factor is absent) causes deficiency. But there's also a passive absorption pathway that doesn't require intrinsic factor at all. This passive diffusion accounts for roughly 1.2% of total absorption, and its bioavailability is unaffected in patients with pernicious anaemia or those who've had gastroduodenal surgery.
At normal dietary doses, 1.2% is negligible. But when you take a high-dose oral supplement (typically 1000mcg or more), that small percentage becomes clinically meaningful. Research has shown that even patients with pernicious anaemia can respond to high-dose oral B12. One prospective study found that after one month of oral supplementation, nearly 90% of patients were no longer deficient, with improvements persisting throughout a twelve-month follow-up.
The practical complication in the UK is that high-dose oral B12 formulations aren't currently available on NHS prescription. If you need supplementation and your GP confirms this, you'll likely be offered injections through the NHS. High-dose oral supplements are available over the counter, but they're not part of standard NHS treatment pathways yet.
UK treatment protocols: what the NHS actually recommends
Understanding the standard treatment helps put private "energy injection" services into context. In the UK, hydroxocobalamin is the preferred form of injectable B12 (rather than cyanocobalamin, which is more common in the US). This is because hydroxocobalamin stays in the body longer, meaning less frequent injections.
The NHS loading protocol for B12 deficiency anaemia involves injections every other day for two weeks, or until symptoms begin improving. After that, maintenance depends on the cause:
- · If deficiency is caused by pernicious anaemia or malabsorption: injections every two to three months, lifelong
- · If neurological symptoms are present: injections every two months after the initial intensive phase
- · If deficiency is purely dietary: tablets may be sufficient, or twice-yearly injections
Notice the distinction. Dietary deficiency (common in vegans, vegetarians, or people with very restricted diets) is the mildest form and often doesn't require injections at all. The intensive injection protocols exist for people whose bodies cannot absorb B12 normally, regardless of how much they consume.
Who is probably wasting money
I say this not to be discouraging, but to be honest. If you don't have a diagnosed B12 deficiency or a condition that causes malabsorption, paying for regular B12 injections is unlikely to give you more energy, better skin, or improved mental clarity. B12 isn't a stimulant. It's a nutrient your body needs in specific amounts, and if you already have enough, adding more doesn't enhance anything.
NHS clinical guidance is clear on this point. For patients with B12 levels in the low-normal range and no symptoms, the appropriate approach is either to monitor annually or to trial oral supplements and recheck levels after a few months. Treating these patients with injections is described in clinical literature as "heavy-handed."
The wellness industry has latched onto B12 injections because they feel medical, they're quick to administer, and the placebo effect of having something injected is genuinely powerful. I'm not dismissing anyone's experience of feeling better after a B12 shot. But feeling better and the B12 being responsible for it are different things.
If you're fatigued, the list of possible causes is long: iron deficiency, thyroid dysfunction, sleep disorders, depression, chronic stress, poor diet quality, vitamin D deficiency, underlying illness. B12 deficiency is on that list, but it's not at the top for most people with adequate dietary intake. A blood test costs less than a course of injections and will actually tell you what's happening.
A sensible approach to B12 supplementation
If you're considering B12 supplementation, here's the sequence I'd recommend:
- · Get a blood test. Your GP can check serum B12, and should also look at folate (since B12 and folate deficiency can coexist and folate masks some B12 symptoms)
- · If your B12 is genuinely low, investigate why. Dietary? Pernicious anaemia? Previous surgery? Gut conditions like Crohn's or coeliac?
- · If you have pernicious anaemia, neurological symptoms, or malabsorption from surgery, injections are appropriate and often necessary
- · If your deficiency is dietary or mild, high-dose oral supplements are likely just as effective and considerably cheaper
- · If your B12 is normal, spending money on injections is spending money on something your body will simply excrete

A note on safety and regulation
B12 injections are very safe. Hydroxocobalamin has an excellent safety profile, and serious adverse reactions are rare. The main risks are injection-site reactions (mild pain, redness, swelling) and allergic reactions in susceptible individuals. Because B12 is water-soluble, the body excretes excess amounts rather than storing them to toxic levels.
That said, the person administering your injection matters. In the UK, aesthetics remains largely unregulated, and vitamin injections occupy a grey area between medical treatment and wellness services. The UK Parliament's Women and Equalities Commission has criticised the Government for "not moving quickly enough" on an aesthetics licensing scheme, with ongoing discussions about tightening requirements for practitioners. Until that changes, the responsibility falls on you to check qualifications and training.
As a registered nurse with NMC registration and an MSc in Advanced Practice, I approach B12 injections the same way I approach any clinical intervention: with appropriate assessment, clear communication about what the treatment can and cannot do, and follow-up if needed. That's the standard you should expect from anyone offering injectable treatments.
The bottom line
B12 injections work when you actually need them. For pernicious anaemia, malabsorption conditions, and neurological involvement, they're essential, sometimes lifesaving, and there's no adequate substitute. For dietary deficiency, they work but so do high-dose tablets, and the tablets are usually cheaper and more convenient. For people with normal B12 levels who want more energy, they don't work any better than placebo, because the problem isn't B12.
The question isn't "do B12 injections work?" but "do I actually need one?" A blood test will tell you. If the answer is yes, we can help. If the answer is no, I'd rather tell you that and save you the expense than take your money for something that won't make a difference.
