Botox for Excessive Sweating: A Medical Use of an Aesthetic Treatment
Botulinum toxin is best known for softening lines and wrinkles, but it has a separate, genuinely medical application: treating hyperhidrosis, the clinical term for excessive sweating. The mechanism is different from its cosmetic use, the results last longer, and for many people this treatment changes daily life in ways that no antiperspirant ever could. In this post I want to explain how and why it works.
The short version
Botulinum toxin blocks the release of acetylcholine, the chemical messenger that tells sweat glands to activate. When injected into the underarms (or other affected areas), it effectively switches off those glands for six months or longer, with clinical studies showing sweat reduction of 82 to 87 percent. The treatment was FDA-approved for severe underarm sweating in 2004, and in the UK it is a licensed option for axillary hyperhidrosis when other treatments have failed. Results typically begin within two to four days and reach full effect within two weeks. For my clients, this is often the most life-changing treatment I provide.
What is hyperhidrosis?
Hyperhidrosis means sweating beyond what is needed for normal temperature regulation. Primary hyperhidrosis has no underlying medical cause; the sympathetic nervous system simply overreacts. It most commonly affects the underarms, palms, soles, and sometimes the face. Secondary hyperhidrosis is sweating caused by another condition or medication, and that needs different investigation.
The impact goes beyond physical discomfort. People avoid certain colours of clothing, carry spare shirts, feel self-conscious shaking hands, and sometimes withdraw from social or professional situations entirely. Standard antiperspirants rarely make a meaningful difference because they work on the skin surface rather than the nerve signals causing the problem. Prescription-strength products help some people, but for severe cases, they are not enough.
How botulinum toxin stops sweating: the acetylcholine block
Understanding why this works requires a quick look at nerve signalling. Sweat glands are controlled by cholinergic sympathetic neurons. When these nerves fire, they release a neurotransmitter called acetylcholine, which binds to receptors on the sweat gland and triggers perspiration. In hyperhidrosis, these neurons are overactive, firing signals far more often than necessary.
Botulinum toxin A works by binding to the nerve endings and preventing the release of acetylcholine. The New England Journal of Medicine confirms this directly: botulinum toxin A stops excessive sweating by blocking the release of acetylcholine, which mediates sympathetic neurotransmission in the sweat glands. With that chemical messenger blocked, the signal simply does not reach the gland. The gland does not receive the instruction to produce sweat, so it stays dormant.
This is important: the treatment only affects the targeted sweat glands where it is injected. It does not interfere with your body's overall temperature regulation. You will still sweat normally elsewhere. The concern some people have about "blocking sweat" causing overheating is understandable but not borne out by the evidence. The underarm area accounts for a small percentage of total body sweat production.
Why effects last longer for sweating than for wrinkles
One of the interesting differences between cosmetic and medical uses of botulinum toxin is duration. When used for facial lines, effects typically last three to four months before muscle activity gradually returns. For hyperhidrosis, the same product lasts considerably longer.
The International Hyperhidrosis Society notes that dryness typically lasts four to twelve months, with some studies finding effects lasting as long as fourteen months. Research published in peer-reviewed journals confirms that the duration of effectiveness for inhibiting sweating is six months or longer, compared with approximately four months for inhibiting muscular contraction. In my own practice, I have found the treatment to be highly effective and generally predictable beyond nine months for most clients.
The reason for this difference is not fully understood, but it likely relates to the different regeneration rates of nerve terminals at sweat glands versus muscles. Whatever the mechanism, the practical outcome is that most people need only one or two treatments per year, which makes this far more manageable than the every-three-months pattern of cosmetic anti-wrinkle work.
What the clinical evidence shows
The efficacy data for this treatment is unusually strong. When used for underarm excessive sweating, botulinum toxin has been shown to result in an 82 to 87 percent decrease in sweating. That figure comes from multiple clinical studies and is cited by the International Hyperhidrosis Society as well as independent research centres.
Patient satisfaction is correspondingly high. Research from Baylor College of Medicine documents satisfaction rates of up to 98 percent across various clinical studies, with minimal side effects and long-lasting beneficial effects averaging six to twelve months.
These numbers align with what I see in clinic. Most clients describe the result as genuinely life-changing. They can wear colours they had avoided for years. They stop carrying spare clothing. Some tell me it has improved their confidence in meetings or social situations in ways that feel out of proportion to "just sweating less," but that is the reality of how much this condition affects daily life.
The treatment process
The procedure itself is straightforward. After an initial consultation to confirm suitability, the underarm area is marked in a grid pattern to ensure even coverage. Multiple small injections are placed just under the skin surface across the affected area. Most clients describe it as mildly uncomfortable rather than painful, and the whole process takes around twenty minutes.
Results start to become noticeable approximately two to four days after treatment, with full effects usually apparent within two weeks. I always schedule a two-week review to assess the outcome and identify any small areas that may benefit from a top-up.
Key practical points to understand:

Regulatory status and why practitioner choice matters
Botulinum toxin A was FDA-approved for severe primary axillary hyperhidrosis in 2004, specifically for patients who had not found relief using antiperspirants. In the UK, NHS guidance confirms it is a licensed treatment for axillary hyperhidrosis, though it should only be used in severe cases where alternative treatments have failed.
Botulinum toxin is a prescription-only medicine in the UK, which means it must be prescribed by a qualified professional after a face-to-face consultation. From June 2025, the Nursing and Midwifery Council specifically requires nurse and midwife prescribers to conduct this consultation in person before prescribing non-surgical cosmetic medicines. This is not bureaucracy for its own sake; it exists because the treatment involves a potent neurotoxin that requires proper assessment and dosing.
The importance of using qualified practitioners with licensed products was underlined starkly in July 2025, when thirty-eight cases of botulism poisoning were recorded in England following suspected use of unlicensed botox-like products in cosmetic procedures. This is an extreme reminder that the choice of practitioner and product is not a detail. At my clinic, I use only licensed products (Allergan's Botox or Galderma's Azzalure) and hold the clinical qualifications necessary to prescribe and administer them safely.
A note on safety and suitability
Not everyone with sweating concerns is suitable for this treatment. During consultation, I assess whether hyperhidrosis is primary (no underlying cause) or secondary (related to another condition), whether you have tried conservative treatments first, and whether there are any contraindications such as neuromuscular disorders or pregnancy.
Side effects are generally minimal. The most common is temporary bruising or tenderness at injection sites. Serious complications are rare when the treatment is performed correctly with licensed products. The toxin does not travel systemically at the doses used; it stays localised to the treatment area and is metabolised by the body over time.
I take a conservative approach to all treatments, and hyperhidrosis is no exception. If I think your sweating might have a medical cause that needs investigation first, I will say so. If I think you might do well with prescription antiperspirants before moving to injections, I will suggest that. The goal is always the right treatment for the individual, not the most expensive one.
Why this medical use of an aesthetic treatment matters
There is sometimes a perception that aesthetic clinics only do cosmetic work, that everything is about appearance. Hyperhidrosis treatment is a good example of why that perception is incomplete. The same product used to soften frown lines can, when applied differently, treat a genuine medical condition that significantly impacts quality of life. It is not vanity; it is medicine.
I trained as a nurse long before I moved into aesthetics, and I still think of myself as a clinician first. Helping someone stop worrying about sweat patches in meetings feels as legitimate to me as any other clinical outcome. The mechanism is clean, the evidence is strong, and the results are measurable. This is what aesthetics can be when it is practised properly.
