Five mistakes first-time Botox clients make

Most people researching their first anti-wrinkle treatment are sensible, thoughtful, and slightly nervous. That combination usually leads to good outcomes. The problems I see tend to come from the opposite end — clients who move too fast, trust too easily, or skip the steps that protect them. This post covers the five mistakes I encounter most often, and how to avoid each one.

The short version

The most common mistakes are wanting too much too soon, choosing a clinic based on price or convenience alone, skipping the face-to-face consultation, not understanding what the treatment actually does, and failing to attend a review appointment. Each of these is avoidable with a little patience and the right information. If you remember nothing else: botulinum toxin is a prescription-only medicine, the results take time to appear, and the person injecting you matters enormously.

Mistake one: wanting too much, too soon

The most frequent conversation I have with first-time clients goes something like this: they have spent months researching, finally decided to proceed, and now want everything addressed in one sitting — forehead, frown lines, crow's feet, perhaps a brow lift, maybe something around the mouth. The reasoning is understandable. If you are going to do it, why not do it properly?

The answer is that anti-wrinkle injections are not like painting a wall. You cannot see the finished result on the day. The onset of effect generally takes two to three days, with the maximum effect observed around day thirty. If a practitioner treats multiple areas aggressively at your first appointment, neither of you truly knows how your muscles will respond until weeks later — by which point any over-treatment is already done.

A conservative first treatment allows your practitioner to learn your anatomy and your preferences. It also allows you to adjust to the change gradually. I would rather a client return for a small top-up at their review than spend three to four months waiting for an overly frozen result to metabolise naturally. The toxin always wears off — generally within three to six months depending on the area, dose, and individual response — but that is a long time to feel unlike yourself.

Mistake two: choosing the wrong clinic

This is where the aesthetics industry lets people down most badly. Botulinum toxin is a prescription-only medicine, which means it must be prescribed by an appropriate practitioner such as a doctor, dentist, or qualified nurse prescriber. However — and this is the part that surprises most people — who actually injects the product is not regulated in the UK. As long as someone works with a prescriber, they can legally inject the toxin regardless of whether they have any medical background at all.

This regulatory gap has real consequences. Between June and August 2025, the MHRA reported forty-one confirmed cases of botulism across England, believed to be linked to unlicensed botulinum toxin products. These were not minor reactions. Botulism is a potentially life-threatening condition that causes paralysis. The MHRA's Criminal Enforcement Unit launched criminal investigations, and the case remains a stark reminder that what you are injected with — and by whom — matters enormously.

At present, there are no legally enforced common standards for training, qualifications, or infection control in aesthetic practice. Voluntary registers such as Save Face and the Joint Council for Cosmetic Practitioners set their own recommended standards, but compliance remains optional. When choosing a clinic, I would suggest asking these questions:

  • · Is the person prescribing your treatment registered with a professional body you can verify — the NMC for nurses, the GMC for doctors, the GDC for dentists?
  • · Is the person injecting the same as the person prescribing, or are they different people?
  • · What product will be used, and is it one of the UK-licensed botulinum toxin brands — Botox, Azzalure, Dysport, or Bocouture?
  • · What happens if something goes wrong? Is there a clear pathway to reach the prescriber?
  • · Does the clinic have appropriate insurance cover?

Mistake three: skipping the face-to-face consultation

Since June 2025, nurse and midwife prescribers have been required by the Nursing and Midwifery Council to consult with patients face-to-face before prescribing non-surgical cosmetic medicines, including botulinum toxin. This is not optional guidance — it is a professional requirement. The same principle applies more broadly: a key regulatory change in 2025 established that all prescribing of prescription-only medicines must be preceded by a mandatory face-to-face consultation with the prescribing clinician.

If a clinic offers to prescribe for you remotely, based on photographs or a video call alone, they are either operating outside the current rules or using a workaround that should make you cautious. The consultation exists for good reasons. It allows the prescriber to assess your facial anatomy in movement, to understand your medical history, to discuss your expectations honestly, and to ensure you are making an informed decision without pressure.

A proper consultation is also your opportunity to ask questions. What areas does the practitioner recommend treating first? What dose are they planning? What should you expect during the onset period? What are the possible side effects? A clinic that rushes this step, or treats it as a formality, is not one I would trust with a prescription-only medicine.

Mistake four: not understanding how the treatment works

Botulinum toxin type A works by blocking the release of acetylcholine at the neuromuscular junction. In plain terms, it temporarily prevents the nerve signal from reaching the muscle, which reduces the muscle's ability to contract. This is why lines caused by repeated muscle movement — such as frown lines or crow's feet — soften when the muscle is relaxed.

What the treatment does not do is fill lines, add volume, or tighten skin. If your concern is volume loss in the cheeks, hollowing under the eyes, or deep folds that remain even when your face is at rest, then anti-wrinkle injections alone will not address it. Those concerns typically require dermal filler or skin-quality treatments like Profhilo. A good consultation will clarify which treatment — if any — is appropriate for your specific concern.

First-time clients sometimes expect immediate results. In reality, improvement generally takes place within two to three days, with the maximum effect observed around day thirty. If you have a wedding, holiday, or important event, plan accordingly. I usually suggest allowing at least three to four weeks before any occasion, with time built in for a review appointment.

It is also worth understanding that the effect is temporary. Depending on the area treated, dose used, and individual response, repeat treatments are typically required every three to six months. The toxin metabolises naturally — your body breaks it down over time, and muscle function returns. This is actually a safety feature. If you dislike the result, or if something goes wrong, it will resolve on its own. Unlike some other procedures, there is no permanent alteration.

Mistake five: not attending the review appointment

The two-week review is not a sales opportunity. It is a clinical necessity, especially for first-time clients. At that point, the treatment has reached its full effect, and both you and your practitioner can assess the result together. Is the movement reduction appropriate? Is it symmetrical? Are you happy with how you look? Is any top-up needed?

I include a review appointment as standard at Visage Aesthetics, and I encourage every client to attend even if they think everything is fine. Sometimes clients notice asymmetry they had not spotted before treatment — one brow slightly higher than the other, for example — and assume the injections caused it. Often the opposite is true: the treatment has revealed a pre-existing difference by relaxing the compensating muscles. A review allows us to discuss this calmly and decide whether any adjustment is appropriate.

The review also establishes a baseline for future treatments. Because I know how you responded the first time, I can adjust dosing and placement for subsequent appointments. Clients who skip the review, or who move between clinics frequently, lose this continuity — and their results tend to be less consistent as a consequence.

A note on safety and honesty

I have worked in aesthetics long enough to know that the industry has a reputation problem, and much of it is deserved. The 2025 botulism cases were not an isolated incident — they were a predictable outcome of a sector that has grown faster than regulation can follow. The government has acknowledged this, and a new licensing scheme is in development, but until it is fully implemented, the burden falls on individual practitioners to maintain standards voluntarily.

I take that responsibility seriously. I use only UK-licensed products with clear provenance. I prescribe and inject myself — there is no separation between the person making clinical decisions and the person carrying them out. I keep detailed records, carry appropriate insurance, and maintain my registration with the NMC. These things should be baseline expectations, not differentiators, but in the current climate they are worth stating plainly.

If you are considering your first treatment, take your time. A few weeks of research now will serve you far better than rushing into a decision you might regret. And if, after reading this, you decide that anti-wrinkle injections are not for you — that is a perfectly reasonable conclusion too.
